There's a set of simple applications that people have put together to create letter that request public records through the Freedom of Information Act. Here's a collection of those sites, with a little commentary on each in part as they apply to requesting public records in Michigan.
This page generates letters for both federal and state agencies. On the federal side, it has separate forms for each of several dozen agencies; on the state side, it gives detailed instructions and references to state statutes for each of the 50 states plus DC. There's sample language in each of several sections to smooth the path of triggering specific exemptions for fees based on public interest.
This is a one-page form. The Student Press Law Center shares an office with the Reporters Committee for Freedom of the Press, so it should be as accurate as the other.
